The High Commission of Malaysia wishes to announce that starting 1 January 2008 all passport applications should be submitted in person at the High Commission of Malaysia in Canberra. The office no longer accept applications for passports by mail.

PASSPORT
With the installation of the new system and equipments, the Passport Office at The Malaysian High Commission in Canberra, is now capable of issuing of machine-readable passports within the same day of application.
To obtain such passport, application must be submitted, in person at the counter of the Consular Division, by appointment, every Monday to Friday, from 9.00 am to 11.00 am. Your new passport will be ready for collection in the afternoon (after 3.30 pm) on the same day.
Malaysian children including new born babies intending to travel overseas must travel with his/her own passport. The parents must apply for a separate child’s passport.
REQUIREMENTS FOR PASSPORT APPLICATIONS
Make certain that you bring along the original and photocopies of all the documents required (except the passport please bring the original), according to the checklist mentioned above. Do not cut copies of documents to size (must be in A4 size)

Application Form IM.42 - Borang Permohonan Pasport/Dokumen Perjalanan). Print the form front and back as a single sheet. Application Form IM.42 may be obtained and downloaded from this website.

Original and a Copy of Malaysian Identity Card (KPT or MyKad version)

Original and a Copy of Birth Certificate / Borang W (For applicant below 18 years old only)

A copy of a valid visa page or a copy of an electronic visa

Blue background passport size photo- Two (2) copies of recent full-face photograph. The High Commission also provides photograph service from 9:00am to 11am, Monday to Friday, with a fee of AUD12.00 for 4 pieces - Cash payment only

Certificate of Evidence of Residence Status - A Malaysian citizen (over 18 years old) who has not retured to Malaysia in the past 5 years or whose Passport has expired for more than six months must provide the above ceritificate which can be obtained from the Australian Department of Immigration and Citizenship (DIAC) in your area.
ADDITIONAL DOCUMENTS FOR CHILDREN UNDER 18 YEARS OLD
- Original and a Copy of Malaysian Identity card of either parent (who signed the consent section)
- Original and a Copy of Marriage Certificate of parents (registered with the Malaysian Government)
- Original and a Copy of either parents passport who signed the conscent on the application form.
- Malaysia Identity Card - for children above 12 years old must obtain identity card
- Parents'/Guardian Consent - Column for consent on page 2 must be signed by either parent - witnessed and certified by the authorized/recognized officer.
NOTE:-

If a non Malaysian parent of a child wishes to renew/obtain a Malaysian passport for the child, he/she must present an authorization letter from the Malaysian citizen spouse authorizing the person to apply for the child's passport.

The office does not accept photographs with other colour background.

INSERTING NEW PHOTOGRAPH FOR CHILDREN OVER 2 YEARS OLD WITH EXISTING PASSPORT
Parents of a child over 2 years of age who have obtained a passport can now apply to insert a new photo of the child at any Immigration Department in Malaysia or at the Malaysian embassies in your residing country.
The following documents are required to insert the new photo in the passport:
1. Either of the Malaysian citizen parents present at the Immigration office
2. Fill up form – IM42
3. 2 new passport size photographs
4. Provide original and copies of:
I. Mykad (if born in Malaysia)
II. Birth Certificate/ Borang W
III. Parents Malaysian identity card
IV. Passport/ Travel documents used when entering Malaysia
PASSPORT FEE (Effective from 26 October 2009)

Adult - AUD 110.00 for 32 pages

Children (below 12 years old) - AUD55.00 for 32 pages
Payment shall be in the form of Cash, Money Order or Bank Cheque and made payable to Malaysian High Commission. Credit Card or Personal/ Business Cheques are not acceptable.

LOSS OF PASSPORT
If you are unfortunate enough to have lost your passport or it has been stolen, you must report the loss to a Police station and obtain a copy of the report. This is the first step towards gaining a replacement.
The High Commission of Malaysia in Canberra cannot issue a new passport to replace a lost passport. All applications have to be forwarded to the Immigration Department in Malaysia who will process your application and notify the High Commission when a the application has been approved.
However, the High Commission may issue an Emergency Travel Document to enable you to travel back to Malaysia to seek replacement. The requirements for an Emergency Travel Document are similar to those for a new passport except that a birth certificate may be accepted as a proof of identity in the absence of Malaysian identity card.
Regaining a Passport after the loss of the original Passport
The following documents are required in support of an application for a new Passport to replace a lost Passport :
1. Form IM42
2. Original and a Copy Malaysian Identity card
3. Original and a Copy of Birth Certificate
4. Original and a Copy of Citizenship Certificate, if
applicable.
5. Original and a Copy of Full Police Report of loss of
Passport
6. Original and a Copy of Marriage Certificate, if
applicable.
7. Two (2) Recent passport size photographs. (Photos
can be taken at High Commission's office in Canberra.)
8. Letter of confirmation from Employer/ University/
school that you are working or studying at present
9. For Permenant Residence of Australia, please obtain a
Certificate of Residence status from the Australian
Department of Immigration in your area (original and
2 copies)
10. For Students, please obtain a letter from the Australia
Immigration Department confirm your status as an
oversea student in Australia (original and 2 copies)
