Child Registration for Birth Certificate

NOTE:

From 16 July 2009, the High Commission of Malaysia will be providing facilities for passport photographs with blue background, on Monday to Friday at 8:30am to 9:30am. AUD12.00 for 4 passport size photographs

Effective 01 January 2008, Child Birth Registration should be submitted in person by either parent (Malaysian Citizen) at the High Commission of Malaysia, Canberra. The office no longer accepts application submitted by mail.

Appointment days: Monday, Wednesday and Friday
Appointment Time: 9:00am to 11:00am
Collection of Document: The same day after 3:30pm

Application for Child Registration and Passport can be done on the same day, and both documents can be obtained before 5p.m. the same day.

If you are applying for Child Birth Registration and the Child's Passport, please lodge the applications before 11a.m to ensure that both documents are ready the same day with the except for the application for paspport for the child whose father's birth place from Sabah/Sarawak the passport has to be referred to the Immigration Department of Sabah/Sarawak for approval prior to obtain the passport. This process may take more than a month to be ready subject to approval by the Immigration Department.  Make certain you make enough photocopies of documents required for each application.  Refer to passport section for all the required documents.

Child Registration (before a year old)

The father of the child must be a Malaysian Citizen at the birth of the child.

NOTE: The Father or Mother who is a Malaysia Citizen needs to sign the application form (Borang D) in front of the officer during submission of the application form (Not witnessed or signed by Malaysian Consulate/Justice of the Peqace or Police Officer).

The presence of the child is not required; however the child's thumb print on the form (Borang D) and child's photographs (at the back of the photos) must be witnessed and certified  by either the Malaysian Consulate or Honorary Consulate of Malaysia (New South Wales, Queensland and South Australia).  

There is no Consulate or Honorary Consulate of Malaysia in Victoria or Tasmania. Therefore, applicants from Victoria and Tasmania can get the thumb print witnessed and signed by either the Justice of the Peace  or a  Police Officer .  

REQUIRED DOCUMENTS ( 2 COPIES OF 2-7)
 

NOTE:

1.      Bring the all the original documents and 2 copies of each documents requested below (The officer at the High Commission of Malaysia  will certify all photocopies documents).

2.     If applying for baby's passport together, kindly provide seperate set of  copies. (Refer to passport requirement)

  1. Borang D (refer to download section)
  2. Child's Australian Birth Certificate
  3. Parents' Malaysian Marriage certificates (If you have an Australian Marriage Certificate, it is compulsory to register your Marriage with the High Commission for Malaysia)
  4. Parents' Birth Certificates
  5. Parents' Identity cards (both sides)
  6. Parents' Passports  including visa page
  7. Child's Father who is Permenant Residence of Australia ONLY must provide Current Certificate of Evidence of Residence Status (Can be obtained from the Australian Department of Immigration and Citizenship)
  8. Three (3) recent passport-size photographs of the child (witnessed, signed and stamped by the same officer witnessing the thumbprints)
  9. Fee of AUD 25.00 (Effective from 2 March 2009) made payable to Malaysian High Commission either by  MONEY ORDER or BANK CHEQUE (Personal cheques are NOT acceptable). The office also accepts CASH.

Child Registration after a year old

The registration takes place after the child turns (1) year old or when the application is received after the child turns a year old.
 

NOTE:

NOTE: The Father or Mother who is a Malaysia Citizen needs to sign the application form in front of the officer during submission of the application form (Not witnessed or signed by Malaysian Consulate/Justice of the Peace or Police Officer).

The application will be submitted to Malaysia. The processing time could take up to 6 months and longer.
 
The presence of the child is not required; however the child's thumbprint on the form (page 5, Borang E) and child's photographs must be witnessed and certified by  either the Malaysian Consulate, Justice of the Peace  or a Police Officer. The father or the mother must lodged the application in person and bring in all the original documents and 2 copies of each document requested below (Does not need to be certified) .

Required Documents (2 copies of 2-6)

  1. Borang E (refer to the downloads section)
  2. Child's Australian Birth Certificate
  3. Parents' Malaysian Marriage certificates
  4. Parents' Birth Certificates
  5. Parents' Identity cards
  6. Parents' Passports including visa page
  7. Child's Father who is Permenant Residence of Australia must provide Current Certificate of Evidence of Residence Status (Can be obtained from the Australian Department of Immigration and Citizenship)

Child Registration by Single Parent

NOTE:
The application will be submitted to Malaysia. The processing time could take up to 6 months and longer.
 
The presence of the child is not required; however the child's thumbprint on the form (page 5, Borang E) and child's photographs must be witnessed and certified by  either the Malaysian Consulate, Justice of the Peace  or a Police Officer. The father or the mother must lodge the application in person and bring in all the original documents and 2 copies of each document requested below (Does not need to be certified) .
 
Required Documents (2 copies of 2-5)
  1. Borang E (refer to the downloads section)
  2. Child's Australian Birth Certificate
  3. Mother's/ Father's Birth Certificate
  4. Mother's/ Father's Identity card
  5. Mother's/ Father's Passport including visa page
  6. Single search document from your state Registry of Births, Deaths and Marriages to show you are  not married in that jurisdiction. This must be attested  by your local State Office of the Department of Foreign Affairs and Trade.
  7. Statutory Declaration signed and witnessed and signed by the Justice of the Peace stating that you are not married in any Australian Territory.
  8. Three (3) recent passport-size photographs of the child (witnessed, signed and stamped by the same officer witnessing the thumbprints)
  9. Fee of AUD 25.00 (Effective from 2 March 2009) made payable to Malaysian High Commission either by Money Order or Bank Cheque. The office also accepts Cash payment.

 

Extraction of birth certificate

Extraction of birth certificate is not done by the High Commission. You can get a an extraction of the birth certificate by sending the application form directly to Malaysia.

Please visit  the Jabatan Pendaftaran Negara (Registration Department) website for downloading the application form at www.jpn.gov.my. Fill up form JPN.LM12 (Application for Search & Extraction).

Documents needed:

1.  A certified copy of applicant identification (passports, birth certificate, etc)

2.  Payment of AUD5.00 in cash which would cover the fee and the return postage.
